Job Description

Summary

Lead patient care as a Clinical Pharmacy Specialist, independently managing medication therapy and applying advanced clinical expertise.

As a Clinical Pharmacy Specialist (GS-13), you will operate at the highest level of clinical practice, independently managing patient care and medication therapy. This role involves comprehensive patient interaction, detailed therapeutic planning, and collaboration with interdisciplinary teams.

  • *Key Responsibilities:**
  • Function as a mid-level provider, independently initiating, modifying, or discontinuing medication therapy.
  • Design, implement, assess, monitor, and document therapeutic plans utilizing effective and economical treatments.
  • Achieve positive patient outcomes through direct and indirect interactions with patients, providers, and teams.
  • Perform physical assessments and order diagnostic tests to evaluate medication efficacy and toxicity.
  • Serve as a consultant for intensive medication therapy management services.
  • Play a defined role in budgetary execution for your program area.
  • *Qualifications:**
  • **Education:** Graduate of an ACPE-accredited College or School of Pharmacy (Pharm.D. or a baccalaureate degree in pharmacy for degrees prior to 2005). Graduates of foreign programs must possess Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification.
  • **Licensure:** Hold a full, current, and unrestricted license to practice pharmacy in a U.S. State, Territory, Commonwealth, or the District of Columbia.
  • **Experience:** Minimum of one year of experience equivalent to the GS-12 level.
  • **Citizenship:** United States Citizenship is required.
  • **English Proficiency:** Must be proficient in spoken and written English.
  • **Core Skills & Knowledge:**
* Ability to communicate persuasively (oral and written) to influence clinical and management decisions. * Expert understanding of regulatory and quality standards relevant to your program area. * Strong problem-solving, coordination, and organizational skills to maximize outcomes. * Expert knowledge in a specialized area of clinical pharmacy practice. * Advanced skill in monitoring and assessing drug therapy outcomes, including physical assessment and interpretation of diagnostic parameters.
