Summary
Seeking an experienced Pharmacist to deliver comprehensive medication management and patient care, ensuring safe and effective outcomes.

- *Key Responsibilities:**
- Review, interpret, and verify medication orders for appropriateness and safety.
- Process and accurately fill medication orders.
- Collaborate with clinical staff, providing expert recommendations on medication therapy.
- Analyze patient records (medications, allergies, labs) to identify and resolve drug-related issues.
- Document recommendations, interventions, and assess adverse drug events (ADEs).
- Manage refill extensions, partial medication supplies, and perform medication reconciliation.
- Provide comprehensive drug information and assist in formulary management (e.g., therapeutic substitutions).
- Participate in medical emergencies and oversee technical staff in medication distribution.
- Communicate effectively to influence clinical and management decisions.
- Apply expert understanding of regulatory and quality standards.
- Monitor and assess drug therapy outcomes, including physical assessment and interpretation of laboratory and other diagnostic parameters.
- *Requirements:**
- **Education:** Graduate of an ACPE-accredited Pharm.D. or Baccalaureate degree in Pharmacy (for degrees obtained prior to 2005). Foreign graduates must possess Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification.
- **Licensure:** Hold a full, current, and unrestricted license to practice pharmacy in a U.S. State, Territory, Commonwealth, or District of Columbia.
- **Experience:** Minimum of one year of professional pharmacy experience involving comprehensive medication management, patient consultation, and formulary oversight.
- **Skills & Abilities:**
- **Other:** U.S. Citizenship is required.
- *Preferred Qualifications:**
- PGY1 Pharmacy Residency or PGY2 Ambulatory Care Residency.
- Board Certification (e.g., BCACP, BCPS).
- 3-5 years of direct patient care experience.
- Experience with telehealth.
- Clinical expertise in areas such as heart failure, diabetes, weight management, and other primary care illnesses.
