Nurse (Consultant/Medical Home Port Team/Patient Centered Medical Home)

This role seeks a dedicated Registered Nurse to provide high-quality ambulatory care and drive continuous service improvements.

• *Key Responsibilities:**
• Plan, implement, and evaluate ambulatory nursing care activities.
• Assist in process improvements to enhance access, quality of care, patient experience, and health service utilization.
• Implement performance improvement initiatives based on facility plans.
• Utilize automated programs and information technology tools to optimize the patient experience.
• *Required Qualifications:**
• **Citizenship:** Must be a US Citizen.
• **Education:**

* A graduate, bachelor's, associate degree, or diploma from an accredited professional nursing educational program. * **or** A PhD or equivalent doctoral degree from a professional nursing program or related medical science field.

• **Licensure:**

* Current, active, full, and unrestricted Registered Nurse (RN) license from a U.S. State, District of Columbia, Commonwealth of Puerto Rico, or U.S. territory. * Successful completion of the National Council Licensure Examination (NCLEX). * *For foreign graduates:* Completion of the Commission on Graduates of Foreign Nursing Schools (CGFNS) Certification Program is required.

• **Experience (if qualifying with a diploma, associate, or bachelor's degree):**

* At least one full year of professional nursing experience (equivalent to GS-9 grade level or above). * Experience must include planning, implementing, and evaluating ambulatory nursing care; assisting in process improvements; implementing performance improvement activities; and utilizing automated programs/IT for patient experience.

• Official transcripts are mandatory for all education-based qualifications.
• Proof of your current RN license and NCLEX passage is required. Foreign graduates must provide a CGFNS Certificate.
• *Exemption for Current Federal Employees:* If you are a federal employee in the 0610 occupational series continuously since September 27, 2017, specific alternative requirements for education may apply.
